Application Form

For nominated first responders and family members, this form is intended to collect individual information prior to attending the retreat.

Note: For groups, each individual first responder must complete this form. At this time, preference will be given to First Responders within 2 hours of Cumberland, WI however other agencies will be considered as spots allow.

Referral Form

For Nominating a First Responder or Family Member to Attend a Rest & Resilience Retreat. This form is intended for individuals who wish to refer a first responder, significant other, or family member who could benefit from attending a Rest & Resilience Retreat. Referrals will be reviewed confidentially by the Founder and designated members of the Board.

Please note: All nominees will be contacted directly by Rest & Resilience Retreat to confirm their interest and provide consent before any participation is arranged.
